• A clear copy of your CDL
• A clear copy of your Social Security Card
• A copy of your Long Form Physical
• A copy of your Medical Card
• Occupational Accident Insurance (included)
• Insurance Application (included)
• W9 (included)
• Tractor Registration (if using your own plates)
• 2290 (if using our plates)
• Tractor Title (if using our plates)
• Tractor Inspection and Receipt (done at a major chain or dealership within the last 30 days)
• Settlement Statement (included)
